| Description | Join Philip Higgins, MSSW, LICSW, Director of Palliative Care Outreach at Dana Farber/Brigham & Women’s Cancer Center and Jane Knowles, MPH, LCSW Clinical Social Worker at The Lilian and Benjamin Hertzberg Palliative Care Institute as they discuss how to identify several models of collaboration, triage and education for times when team census is high, as well as strategies for effective case finding and outreach when census is low.
