This is a gem of a book. Derek Doyle needs little introduction, as he has been at the forefront of the development of hospice and palliative care in the UK and internationally for many years, not least as senior editor of the Oxford Textbook of Palliative Medicine. Now, in this delightful book, he treats us to his considered opinions, and very thoughtfully considered they are, about the fundamental issues involved in hospice and palliative care.

The memories are a rich tapestry of stories drawn from twenty years of practice - some very amusing, some very sad, all recognisably genuine. Loved ones misguidedly believing they were shielding the other from bad news. The remarkable changes in attitudes and behaviour that we see in those terminally ill. The young woman who refused to die until her husband and mother were reconciled - and many more. Doyle tells his stories well and it is an enjoyable book to read.

The musings, imperceptibly interwoven with the stories, are the essence of this book. Here are Doyle¹s very carefully considered opinions on a range of issues that are fundamental to hospice and palliative care - the individuality of patients, the meaning of spirituality, that what is not said may be more important than what is said, the need to empower patients to complete unfinished business, the merits of home care, the inadequacies of modern medicine in dealing with the terminally ill, and much more. And he does not shy away from the question of legalising euthanasia.

If you are involved with hospice and palliative care, you need to read this book. I was struck by an awesome devotion to patients, not just as a professional carer, but also as a friend. You may not agree with everything Doyle says, but you will be enriched by being challenged to think about these fundamental issues, to the benefit of your patients.

Why the title "The Platform Ticket"? Well, you'll have to read it.
