Purpose
The International Association for Hospice and Palliative Care (IAHPC) will give one annual recognition award in the amount of US$2,000 (two thousand US Dollars) to an Institution that has implemented and is actively operating a successful palliative care unit, program or department. The award is directed towards both public and private nonprofit institutions and has the following goals: 1) to strengthen health care services for patients with incurable illness and their families in order to improve their quality of life, and 2) to promote the formation of alliances and work in cooperation among the health, academics and/or governmental institutions.
Objective
To support the development of models of care for palliative care throughout the world.
How to apply
The application should be double spaced and submitted in English. The application and proposal are not to exceed fifteen pages in letter size.
It includes:
A description of the institution program or department.
The curriculum vitae of the person who is responsible for the service/program,
Letter from the Institution: the legal authority of the institution providing palliative care services should provide a signed letter
Selection Criteria
The institution will be selected according to the fulfillment of the following criteria:
At least the person in charge of the unit, department or service is a current member of IAHPC. Membership can be processed trough our secured server in our website at www.hospicecare.com
Institutions which have not received the IAHPC award in the past;
Effective availability of oral opioid analgesics;
Effective availability of medications needed to provide control of symptoms other than pain;
The existence of appropriate infrastructure to provide adequate care for the patients and their families;
The existence of appropriate treatment protocols, including participation of professionals from various health care fields (interdisciplinary care);
The level of involvement of the institution with the community;
The extent to which the proposed initiative will be used as leverage for broader changes in the health care system and in the community;
The quality, clarity and completeness of the application
Applications received before December 31st. Applications received after this date will be considered for the following year.
IAHPC will convene an advisory committee to review the proposals and select the recipient of the Institutional Award. Winners are announced in January/February of the following year.
NOTE: Programs and institutions affiliated with individuals who are either members of the Board of Directors or employees of IAHPC are not eligible to apply.
